The UplandCare project is a model project in the rural communities of Willingen and Diemelsee, funded by the Hessian Ministry of Social Affairs and Integration and the Hessian long-term care insurance providers. The aim of the model project is to provide the best possible support for people with dementia living at home and their relatives and to stabilize their home-based care arrangements. In the model project, existing support services are linked, affected persons are individually and continuously advised and accompanied by a case management and the local population is raised in awareness of the topic of dementia. The health network PORT Willingen Diemelsee e. V. developed and implements the model project. The working group Care Structures at the DZNE site Witten is in charge of the scientific evaluation. This mainly comprises a process evaluation, but should also provide initial findings on the effectiveness of the model project.
